Vanderbilt Medical Center Emergency Department Study

Vanderbilt Medical Center, Nashville, Tennessee
Dr. Seth Wright, Associate Professor of Emergency Medicine - Investigator

Influenza is a major problem in the emergency room, creating overcrowding and straining resources. There are over 5,000 Emergency Room departments (EDs) across the United States and they handle 100 million visits per year. Vanderbilt Medical Center ED has over 55,000 visits per year. Respiratory diagnoses are the number 1, 2 and 6 most frequent diagnosis in the ED.

Expensive and time-consuming work ups are frequently done on these acutely ill patients. Investigators at the Vanderbilt University Department of Emergency Medicine conducted a study to determine if testing for influenza at the triage desk would decrease charges and inappropriate testing in these patients.

The purpose of the study was to determine if using a quick influenza diagnostic test on patients, before they were examined by a physician in the emergency department, could improve the quality of care and lower costs.

Study Parameters:
Patients with febrile respiratory illnesses were randomized to have either testing with the ZstatFlu test or no flu testing followed by standard ED care. The selected patients were 18 to 60 years old and in otherwise good health.

Results:
Patients tested for influenza with ZstatFlu had a 24% decrease in their emergency room charges, saving an average of $161 per patient. Savings were achieved for the test group by reducing the overall number of laboratory tests and chest x-rays. Patients tested with ZstatFlu spent an average of 30 fewer minutes in the emergency room.

Conclusions:
The Vanderbilt Medical Center study concluded that in adult patients, the use of the ZstatFlu test to quickly diagnose influenza A and B in the emergency room decreased ED charges, laboratory tests and patient time spent in the emergency room.
