Enclosed populations (nursing homes, hospital ICUs, colleges, jails, etc.) pose particular public health risks. There is no 'real time' surveillance of influenza onset each season. Currently the ultrashort incubation period of influenza (1-4 days), coupled with the lack of timely outbreak warnings, will prevent isolation or limitation of its spread. This results in a full-blown endemic by the time it is realized influenza is present in a community. Consequently, protection of the person in an institutional setting is provided only by previous immunization. Surveillance testing in the captured population will allow for early detection and intervention, with antivirals to control infection. Clinical studies are planned for this application and its protective outcomes.
